Effect of a Practice-wide Anti-TNF Proactive Therapeutic Drug Monitoring Program on Outcomes in Pediatric Patients with Inflammatory Bowel Disease
Reports on the feasibility and effectiveness of translating proactive, antitumor necrosis factor (TNF) therapeutic drug monitoring (TDM) for inflammatory bowel disease into practice-wide quality improvement (QI) are lacking. The study aimed to determine whether a TDM QI program improved outcomes at a large academic pediatric gastroenterology practice. The primary outcome was sustained clinical remission. The adjusted risk of developing high titer anti-drug antibodies (ADAs) was lower in the post
Characteristics and Long-Term Outcomes of Pregnancy-Onset Inflammatory Bowel Disease: A Case-Control Study
Inflammatory bowel disease (IBD) frequently affects women during their reproductive years. The impact of pregnancy in patients with established IBD has been widely studied, the characteristics and outcomes of patients who develop a new diagnosis of IBD during pregnancy or the postpartum year (“pregnancy-onset”) are not well characterized. Pregnancy onset disease had a 4-fold increase in the risk of hospitalization. Patients with pregnancy-onset IBD more commonly develop ulcerative colitis and ha
Phenotype and Natural History of Inflammatory Bowel Disease in Patients With Concomitant Eosinophilic Esophagitis
Eosinophilic esophagitis (EoE) is more common in patients with inflammatory bowel diseases. Cases consisted of patients with IBD and EoE, with both diseases diagnosed using established criteria. Patients with EoE presented more commonly with dysphagia and had evidence of esophageal rings. No differences in medical or surgical therapy for IBD between the 2 groups. Patients with IBD-EoE are diagnosed at a younger age and more likely to be males.
TRIM21 Is Decreased in Colitis-associated Cancer and Negatively Regulates Epithelial Carcinogenesis
Tripartite motif-containing (TRIM)21 is reported to be associated with the regulation of immune response in the gut mucosa. We analyzed TRIM21 expression in tumor tissues from patients with colorectal cancer and ulcerative colitis-associated cancer. TRIM21 expression is decreased in colitis-associated cancer and negatively regulates intestinal epithelial carcinogenesis. It may serve as a novel therapeutic target for CAC therapy.
Prevent Runny Nose
A runny nose is a symptom of many conditions. It’s characterized by mucus draining or dripping from the nostril. Mucus is a protective substance produced by the mucous membrane, a type of tissue lining the nasal cavity. Mucus moistens the air you breathe, and it acts as a barrier to keep dust, pollen, and bacteria out of your lungs. Your nose produces mucus every day, yet you probably don’t notice because it mixes with saliva and drips down the back of your throat. Sometimes, irritation
The ACE (Albumin, CRP and Endoscopy) Index in Acute Colitis: A Simple Clinical Index on Admission that Predicts Outcome in Patients With Acute Ulcerative Colitis
Intravenous (IV) steroids remain the first-line treatment for patients with acute ulcerative colitis. 30% of patients do not respond to steroids, requiring second-line therapy and/or surgery. Nonresponse was defined as a requirement for second-line therapy or surgery. 78.1% (25 of 32) of patients with concurrent CRP, albumin, and increased endoscopic severity did not respond to IV steroids. This combination of parameters identifies on admission a high-risk population.
Goiter
A goiter (GOI-tur) is an abnormal enlargement of your thyroid gland. Your thyroid is a butterfly-shaped gland located at the base of your neck just below your Adam's apple. Although goiters are usually painless, a large goiter can cause a cough and make it difficult for you to swallow or breathe. The most common cause of goiters worldwide is a lack of iodine in the diet. In the United States, where the use of iodized salt is common, a goiter is more often due to the over- or underproduction of t
